A “practice” consists of one or more healthcare professionals who can view the same group of patients. When you have a professional account in the LibreView system, your account is associated with at least one healthcare practice (even if that practice is just you). If you choose to invite other care team members to your practice, they will be able to see the same patient reports that you see for that practice.
Your Practice ID is a unique code that you can give to patients if you would like them to share their data with you on the LibreView system. Patients can manually enter your Practice ID into their LibreView settings to enable sharing.
To find your Practice ID, log in to your LibreView account. Click on the navigation icon in the upper right corner of the screen. When the side menu opens, you will be able to see your Practice ID.
Log into the LibreView system. Click the 'My patients' icon in the upper left corner. Then, click the search icon (magnifying glass). Use the green search bar to search for the patient you would like to view. You may also sort the patient list by clicking on the Status column to see the most recent uploads from your patients in chronological order. Click on the patient’s name to view their recent data.
You can invite patients to upload from home by sending an email invitation from the LibreView system or by giving the patient your Practice ID. Learn more: Set Up Remote Monitoring
